I started taking digital pictures of stuff like that in February of 2002.
Print out what's needed on the laser printer.
Works like a charm.

Here's what I do-
Find a copy center that has a "guillotine"--a device that slices off the binding.
Scan the de-bound book.
Have the cookbood rebound with a spiral binding.
Done it for a dozen books--works really well for me.
