How to create a lead management system?

So my brother owns an auto transmission shop. He uses a paper lead form for new customers. These paper forms pile up, and are never looked at again.

I'd like to create an electronic (web?) form to be filled out on a tablet, phone or computer, that can be saved. I'd like the form info to go into a database that can easily run reports against and be followed up on.

Any suggestions?

google forms for simple, or salesforce.com for dedicated app.

For the data COLLECTION, I second Google Forms. It would take all of ten minutes to set up.

However, Google Forms/Spreadsheets currently lacks a lot when it comes to extracting that data into reports, other forms and formats, etc. They may be ways, but they are not as "databasy" as I'd like presently.

Yes. Fast and painless to sign up for a Google account. That gets you G Suite, Gmail, and the uncountable other Google services.

If it's worth it to the shop, you could get G Suite for Business, and be able to assign their own domain, create email addresses using that domain, etc.

Sugar CRM offers a free, opensource version of their software. If you've ever set up a LAMP server it's pretty painless to setup and configure.

I have a Google account. I don't see anywhere to do Google Forms. When I "Google" it, I get G Suite for Businerss, with charges...

I*'m reading this in a primer for Google Forms:
Note: You can store results from multiple forms in one spreadsheet; each form's responses will be saved to a separate sheet. You cannot, however, save multiple forms' responses to a single sheet.

So each time the form gets filled, it's in a separate sheet? I'd like the responses to be in one sheet on separate rows...

Not stupid at all!
A single forms responses are stored as a row in a single sheet. Multiple forms can send data to multiple sheets (each response being a row)... though to be honest, that feature is new to me.

Forms is both tremendously powerful, and frustratingly limited, at the same time.
Once the responses are in Sheets, you can manually manipulate that data however you wish, but doing "database" like things with the data is still very difficult within the Google-verse. I hope that Google chooses to start an online database of some kind. That would be BIG.
