PowerBook: click click click
#1
but I don't think it's the hard drive. Disk Utility says that S.M.A.R.T. Status is verified and I have a recent CCC backup on an external drive

PowerBook G4, 15"

Click Click Click seems to come from under the power button (right side near the screen). It appears the HD is located under the track pad http://www.ifixit.com/Guide/Mac/PowerBoo...Hard-Drive. The noise does not change when I read-write data. It just clicks about once per second. I think it's actually 2 clicks followed by 1 second pause.

if I put the PB to sleep, the noise stops, now I woke up the computer and the noise is gone, however I know it will come back since I heard it now for a few days. I wonder what it is, since it did not do this until a few days ago. I had this powerbook for about 30 months now.

I had a disc with the same problem-- never gave a SMART error of any kind (while a different disk did). And then it was gone completely. The freezer trick didn't work.
Your hard drive is toast-- back up now.
