here is my experience.

I believe over the air is best quality (if signal is good). Then Comcast cable TV is OK, more compressed but still OK in my experience. Then next comes streaming, for example DirectTV Now (which finally works a little better).

I would expect FIOS to be at least as good as Comcast cable TV, but, surprise surprise, I was visiting a friend last night and his TV was on CNN and I was surprised how bad (pixelated) the image looked. he has an LCD tv, while I am used to watching a Plasma TV. I think that ma affect how colors look like, but I doubt it can explain why the image was pixelated. It really looked like DirectTV Now was in December when they had all those streaming issues.

The image from FIOS is rock solid excellent in my place on every channel. Even standard def stuff looks good. However, the quality of the picture can be impacted by several things. The quality of the connections between the ONT and the rest of the FIOS hardware, the TV itself, the channel that day, even the ONT box itself (if it's an older model), you name it. We had awful quality on a few channels and it turned out the fiber connection inside the ONT box was skewed. That was fixed and the picture quality on the shoddy channels transformed and became awesome.

Your friend needs to contact Verizon to have them troubleshoot the picture quality issue. It's positively unusual. Definitely something happen that is out of the ordinary.

My dummy/FIOS story:
I recently purchased a 4K TV. I set it up and was very disappointed by the picture, it was worse than the HD TV it replaced. I googled optomizing the settings for this particular set and was just about to put it back in the box when I realized it was on 480p.

My first couple of months with an HD TV I was wondering what all the hoopla was about. I was distinctly unimpressed.

I had a visit from the cable tech and he kindly informed me that I needed an HDMI cable, which the previous tech had not bothered to tell me. Voila! That's what the hoopla was all about.

I'm just lucky that the second visit wasn't 5 years in.

Yes, check resolution settings on both the TV and cable box.

My Xfinity cable box defaulted to 720P when I first plugged in the HDMI cable. I had to dig through the settings to get it to output 1080. One of the preference settings that is easily accessible, allows you to default all search and TV listings to display only HD channels. If I enter in a standard definition channel number--(anything under 102 on my current system) if there is a companion HD channel--it will automatically switch to the HD channel.

You can change a setting that automatically tunes to the HD version of a channel if one is available. Just don't try to explain why tuning to channel '7' takes you to '507' and that they're the same channel but one is SD and the other is HD and...

Added bonus is for channels in the 500s, if you press just the last digit, or two, 1-99, it will tune to the HD channel.

Of it is pixilation, the issue is signal strength/quality and the connections would need to be checked,including any splitters. We had this issue when an outside splitter was corroded. It should be easy to see by naked eye whether the issue is resolution or signal quality (actually, if it is signal quality, SD channels may look better than HD).

And just restarting the box may do wonders (pulling it out of the wall or doing a soft restart through the menus).
