Sometimes a local network merely gets synced to a single machine on it, and if/when that machine doesn't do time right, all synced machines are also off (like MRF). When a real Network Time Server is polled over the WAN, the time is synced to a better approximation of the real time. Then there's the combo/hybrid routine where only the main local machine checks the WAN, and all other local machines are synced to it, and there may be some latency involved, that why not all time is equal.

And then there is Windows. Its process for checking time servers can fail and stop running, or not apply the correction needed after querying a server. I haven't looked at bug reports related to that recently, but past versions have also had a "misfeature" where if the difference between the local time and the time server was too large it would be ignored.
