My vaccine card says “Stephan” rather than “Stephen”

What a royal PITA. I got my first vaccine five hours away in my hometown, and I guess in theory I am supposed to go back there and ask for a corrected vaccine card.

when did this happen and when did you notice this?

which vaccine did you get? if you took a 2 shot, did you ask at the second visit?

On my card the first date was 5/1/21 and the second date was 5/22/20, she immediately wrote a 1 over the 0

Might be easier to legally change your name!

Yes, you should get this corrected - possibly with a phone call. What really matters is if the state has your name correct in their records. I would check the state's records first - thru an online portal?

Not sure it matters. Every vaccine place will vaccinate someone even without ID or a name. Just give you a blank card with the brand, date and lot number so the patient can monitor for any recalls or such

Now that you mention it, I just checked mine and immediately remembered that I was surprised I had to fill out my name myself. Didn't even notice until I got home.

I think you should legally change it to match your card.

I didn’t notice until weeks later, and the appointment for a second shot was at another location.

What Mr645, wurm, and Lux said. It was fill in your own blanks.

We signed up online, so the info w/ the state registry is what we typed in ourselves, and that's what shows in the state's database. Check your state's database first, and see what info's hanging out there; you may be OK.... or if it's wrong, change it there.
Good luck.

My CARD is correct, but they entered my name wrong in "the system"... so any electronic vaccination proof has my mis-spelled last name on it. angry villagers smiley

I filed a request for correction almost two months ago now... so far, nothing.

From a real 'Stephen' to a clear fraud!!!!

I think this just means your MRF handle is WRONG! Get out of here!

Honestly, I think you are fine. Just send them a note and ask for options.

In addition, I was able to download my proof of vaccination from my medical records.

There is no national vaccine database.
"It’s important to know that the CDC does not store individual vaccination records. This is because vaccination sites and provider offices don’t report this information to them. Therefore, the CDC cannot provide you with a record of your COVID-19 vaccination."

Each state may have their own system. Florida for example has the SHOTS system. However they only release vaccine data to the individual requesting it. No one can request vaccine data on someone else.

"Submit your request directly to Florida SHOTS: You can request your COVID-19 vaccination records directly from Florida SHOTS by filling out the Florida Department of Health form – DH3203 Authorization to Disclose Confidential Information form online, electronically sign and submit it.

To fill out the form online and submit
Note: This form is for use only by individuals aged 18 years and over to request their own COVID-19 vaccination record. DO NOT use this form to request records for another individual. Each individual is responsible for their own request. Currently, records of minors (aged 17 years and below) are not available through this request portal.

If you brought that to me at a clinic, I’d gladly make you a new card with your actual name on it, and your original lot numbers.

You had to drive 5 hours for a vaccine shot? Is this normal in rural areas?
