I have been sold on taking omega-3 fish oil

I’m leaning toward Carlson pills with lemon flavor. I doubt if I would choke down liquid fish oil. Does anyone have any reports to tell about your success or failure with it?

I do the Carlson lemon-flavored liquid. I swig it from the bottle, my best estimation of 2-3 teaspoons. Works fine. The only trick is to keep the bottle inside your lips, and not let the fish oil get on the outside of your lips--if you do, you'll smell fishy until you wash it off. Oh, and I don't get any fish burps.

The generic didn’t affect my triglycerides at all. My cardiologist put me on lovaza. The capsules work well. There are a few occasional fish burps.

Old Walmart fish oil and buffered niacin lowered my triglycerides from 1,000 to 500 within a month twice.

There are studies on both sides of the question-REDUCE-IT comes out differently than STRENGTH, for instance.

There is no definitive answer at this time. My guess is that it probably varies according to the individual's genetics, diet, exercise, and other factors. Here's a fairly non-biased overview:

All that said, I agree with Lew on a key point--if your blood markers don't show a measurable difference, it's a questionable investment.

Jeez Dennis, triglycerides that high are in the reserve a room in CICU territory. Even the lower reading is 5x the recommended level. Time to start taking better care of yourself.

... so, maybe at best a modest benefit, but unlikely to cause any harm. Also, as pointed out, it may depend a lot on things like pre-existing conditions. For example, a forum member mentioned that they have familial hypercholesterolemia:

... and combining with a statin (yes, I know some here are really against them) may be beneficial:

Me personally, I switched to the Costco wild Alaskan fish oil recommended here many times, and I can see why it was so highly recommended... very reasonably priced, no obvious fish burps, easy on the stomach, and my cholesterol is decent. (This post made me go back and look at my numbers, and my last test results were at an all-time low for me!)

Have a look at the results there. There were some studies which supported using specific formulations of Omega-3 Fatty acids which reduced the rates of some cardiac events but had no effect on severe events and premature death. But these were not testing Fish Oil.

An example:

"A 2020 Cochrane review of 86 randomized controlled trials published between 1968 and 2019 found that 0.5 g/day to more than 5 g/day LC omega-3s for 12 to 88 months in a total of 162,796 participants reduced serum triglyceride levels by about 15% and slightly decreased rates of cardiovascular mortality and coronary heart disease events [67]. However, the supplements did not affect all-cause mortality, cardiovascular events, stroke, or arrhythmia."

The weakness of all the data on this page is the qualities of the studies and you'd need to read everything to know what's good and what's not, which may not be worth our time. Heh, and never mind p-hacking which all science is subject to.

I have high Triglycerides and take Niacin because it reduces my Trigs by more than 80%. However there's no evidence that says Niacin helps in any way to prevent a cardiac event. I'm not counting on it to do anything, it's merely addressing a bad number and if I'm lucky, maybe help somewhere.

The same goes for Fish Oil, just be aware that there's little to no consistent evidence that it affects medical outcomes.

My experience is that most skeptics have an axe to grind, but want to be considered to be neutral purveyors of facts. I understand that! There are a lot of quacks out there! I help prosecute some of them!

But if someone makes a big deal of debunking stuff, then I really wonder why they're doing that.

I've used it for about 10 years, based on a recommendation from my eye doc. I've had difficulty with dry eye after several eye surgeries. My condition has greatly improved since I started taking it so it's a keeper for me. Any other benefits would be gravy at this point and I'll gladly take 'em.

I'd put it this way instead: Many skeptics are a-holes and full of themselves. Convinced of their own superiority and utterly unafraid of sharing that with you. As are many people who call themselves skeptics but don't actually apply the science to their sacred cows.

The people at SBM (the link) seem to have a lot less of that or pretty much none of it. I sometimes don't agree with some of their emphases but they're usually pretty much spot on with the medical analysis.

Unfortunately one of them is so long-winded that I haven't finished one of his posts in many months. It's too bad as the topics are interesting. Just too much, needs an editor.

Meaningful impacts on cardiac events have not been reported in recent studies.

This morphed into “ there are no benefits to taking fish oil/ Omega-3 supplements“ from some quarters.

However, other benefits to taking them have been shown in multiple studies as pointed out in my link.

I have been told by numerous MDs that there are essentially only 2 downsides. Fish burps are possible, and they aren't free.

The filtration process removes any mercury, so it is a cleaner source than actual whole fish. Now, yes, there are plant sources for Omega fatty acids.

This assumes from a reputable source.

We take it despite the studies that suggest there is little or no benefit. Because those are population measures and individual outcomes can vary so they might benefit us, individually. Also, "society" might want to reduce unneeded supplements because they are expensive, in a societal sense. But, as individuals we can afford them. So, we'll stick with them until/unless they are shown to be damaging.

There is another good reason for taking fish oil supplements.

People in the Western world seldom consume enough fish to provide the omega-3 fatty acids used to support healthy brain function and production of new cell membranes.

We can get it from other sources, and as with most of these things the evidence that might link healthier outcomes specifically to taking supplements is lacking, but it's another reason why it might be good to take a supplement.

I think this is a "doesn't hurt, so why not?" situation.

When I was in practice, the prevailing thoughts were that the main reason to lower triglycerides in the thousands was to prevent triglyceride induced pancreatitis. Cardiovascular benefits were in doubt, based on the quality of data available at that time. Pancreatitis is a pretty severe condition.

Well, I would guess that most of that was due to the niacin, whose efficacy in lowering blood lipids is well recognized. Side effects are usually the limitation, but it sounds like you’re doing well with that.

My doc had me on fish oil capsules 1000mg.
I could smell it when I sweat.
Told my doctor, he switched me to Flaxseed Oil 1200mg. No smell. Same Omega3.

For inflammation, especially joint, you need EPA and DHA omega 3s, over a gram a day. To get that from flax seed oil, you need multiple tablespoons a day.

Flaxseed supplies about 15 to 20 percent of the omega-3 as the same amount of fish oil. So you need maybe 5X the amount of flaxseed.

If you are concerned about PCBs, remove the skin and dark flesh from your salmon, and cook it so that the fat drips off -- thus reducing PCBs by 20%-30%.
