I had a spontaneous crack develop on a 2x2 double pane window. The manufacturer is out of business so I couldn't order a replacement directly. I could have had one built for me but it was very expensive. So, I took the panel out and carefully removed the cracked pane from the panel (it really just took some heat and a stiff putty knife and 10 min). I then bought a replacement piece of regular glass from Lowe's and had them cut it to size and used clear silicone caulk to put it back on the panel. I'm sure I'm not getting as much efficiency since it doesn't have whatever the gas was but it looks just fine and cost about $15 for the glass.

Edit: I've also discovered that when the seal fails and I get condensation, I can use a very small glass drill bit to put a small hole at the bottom and another at the top of the inside pane of glass. The sun hits it and the condensation evaporates out. Doing it as soon as the condensation is noticed meant that there was no mineral deposits left on the window, clouding it. Again, I'm probably not getting the efficiency that I had when the windows were new but this is a no-cost fix for condensation.
