Sometimes the aggravation balances out. I had a procedure 9 months ago that had a copay of $150. This particular facility bills for the copay after receiving payment from the insurance company. I called this morning about it because I never got a bill. After digging, they found the record and told me that I had a zero balance. PUNctually is a real adverb, and doesn't need a comma before it. Dismayed is not an adverb in the example sentence, and that's why it needs the comma. You could use "noted a dismayed Tom" or "noted Tom with dismay." Whether these two variants are true Tom Swifties is a matter for discussion.
