If you're connecting via the "personal hotspot" option at the top of the WiFi menu, the Mac has to be able to connect to Bluetooth as well as WiFi, and be signed into iCloud with the same Apple ID, and be able to verify the user's credentials through iCloud while it's setting up. There are a lot of ways for it to fail.
